Background
The T Project is Singapore’s first social service organization for the transgender community. This independent community initiative was set up by Miss June Chua and her late sister Miss Alicia Chua in 2014. They realized the many gaps in the existing social service sectors, which did not address the needs of Singapore’s transgender community.
Aware of these challenges, they opened a shelter that would serve as a temporary safe haven for anyone who was homeless in the community. The residents are welcome to stay at the shelter, until they can find alternative housing or a more permanent place.
The shelter has since expanded through a collaboration with another Social Service Agency in 2021. The T Project shelter is the only social service in Singapore that exists specifically to cater to the transgender community and transgender people living with HIV.
Our shelter also provides suitable referral support to our residents to meet their employment, social, healthcare, shelter, and emotional needs. Working with a large pool of volunteers, including social workers, psychologist and counsellors, The T Project has provided case work and counselling support for our residents.
Position Summary
The T Project is looking for a full time Social Worker to be located at our new shelter location. As part of our expansion, we hope to provide more consistent support for our residents.
The T Project will provide appropriate supervision and training to best support your development as a social worker.
Job responsibilities
Manage referrals and applications for admission into the shelter
Conduct assessment, planning and intervention for cases
Engage clients in a professional and therapeutic manner to understand needs, and to build rapport with clients
Document and evaluate case outcomes
Identify, develop and review appropriate intervention strategies, in consultation with supervisor/ programme manager
Organize, coordinate and conduct case conferences with external agencies
Coordinate and monitor delivery of services with other community agencies
Plan, implement, monitor and evaluate therapeutic group work for clients
Identify and engage clients, for relevant group work or programmes/activities
Assess, develop and implement discharge planning for clients, including referring to appropriate community partners to provide continued support/services
Follow up with and monitor post-discharge status and outcomes of clients, and liaise with agencies on progress of clients
Collaborate with community agencies in planning and implementation of programmes and activities
Maintain daily records, update case files of clients and prepare reports for administrative and intervention purposes
Conduct counselling sessions for clients of Alicia Community Centre.
Candidate Requirement
Technical Competencies:
Accredited social worker with a bachelor’s degree in Social Work from an accredited institution / Has a post-graduate Diploma in Social Work and;
Fresh graduates are welcome to apply-No experience required.
